Prior to starting therapy, a client information packet needs to be completed and returned. Along with gathering new client information, this document discloses details regarding my professional credentials and business policies, as well as rules and regulations specific to counseling.

This document will be provided after initial intake by phone or email.


Fees

15-minute initial consultations are free. These meetings can be either by phone or in-person and are a great way for you to meet Josh and see if he might be a fit for you or your teenager's needs. 

Standard Individual Adult, Child, Family and Couples fees are $160 per 50-minute session. For 80-minute sessions, the fee is $240.

Payment is due at the time of therapy. Cash or check (payable to Belay CC), as well as major credit cards, FSA, HSA and Paypal are acceptable forms of payment.


Cancellation Policy

Full session fees are collected for cancellations made within 24 hours. Cancellations made outside this 24-hour window may be rescheduled for any reason with no penalty. This cancellation policy does not apply for serious or contagious illnesses and emergencies. Please communicate a cancellation more than 24 hours prior to your appointment.


Insurance

Belay Counseling & Coaching, LLC. does not bill health insurance providers. However, some insurance companies will accept receipts for psychotherapy for your reimbursement. Before beginning therapy, please contact your health care provider to obtain any pertinent requirements for reimbursement.    


Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
